The CSV includes a request ID, project, round, version, video timecode, requester, request timestamp, requested change, owner, status, completion description, completion timestamp and approval reference. Open it in your spreadsheet app and replace the clearly marked example.

Use an agreed time format. A video position such as 00:18.500 is not a calendar timestamp. If your team uses frame timecode, record the frame rate and timecode convention alongside the sheet.

Weak request: “The beginning feels wrong.” Actionable request: “V2, 00:18.500: replace the opening title with the approved spelling in the brief.” Keep ambiguous creative feedback, but ask what outcome the client expects before calling it complete.

Completion example: “V3 replaces the opening title; client confirmation pending.” This records your action without inventing an approval. If a request is deferred or declined, record the reason instead of marking it applied.

Agree on the round being reviewed and a feedback cutoff outside the tracker. Group notes by that round and collect contradictory requests before editing. A tracker does not amend a contract or automatically establish an extra charge.

When the cut changes duration, do not silently reuse an old timecode as though it points to the same moment. Keep the original version reference and add the new location in the completion description.

A sheet works for a small batch of notes. A video review room becomes more useful when reviewers need to seek directly to the moment or annotate the image. Use the same discipline in either place: identify the version, describe the change and separate applied status from approval.
